Summary: This film depicts the work of the staff and residents of Horizon House, a community-based mental health facility in Philadelphia. The program features clinical subjects, staff, and residents, and stresses the importance of community involvement in planning and meeting the needs of all. The challenges facing the patient about to be discharged from a mental hospital are discussed, including housing, employment, and loneliness. The planning done today is compared and contrasted with that of fifteen years ago. The program focuses on the establishment of Horizon House, first founded as a social club. Horizon later added rehabilitation services, residential services, social casework, and sheltered workshops. In a recent innovation it became associated with the Jefferson County Community Medical Center as a community mental health facility, with links to other health and welfare resources in the area. The program then focuses on the work of the Horizon House teams, comprised of professional and nonprofessional staff members and community members. These teams make home visits to patients and are concerned with housing, money, food, clothing and friends, and assistance in problem solving.
